Housing Models
24 Hour Residential Model
Our 24-Hour Residential Supports Program provides individualized supports in a residential home setting to adults in various communities in Hampden County. Supports range from assistance with all aspects of daily living, promoting commu-nity inclusion, building and maintaining meaning-ful relationships and promoting independence. Houses are designed to foster independence, indi-vidual choice, voice, human rights, and personal growth.
Individual Options Model
In our Individual Options Program, we support adults who reside independently in the community, yet benefit from some specific areas of support to live a fulfilled life. Our Individual Options Team provides customized support designed to maximize an individual’s potential, increase their independ-ence, and promote meaningful connections in their community.
Shared Living Model
In our Shared Living Program, an individu-al is matched with a shared living provider based on shared interests, support needs, location and related other areas to create the best match possible. An individual may be welcomed into the provider’s home or the two may identify a home together. Individu-alized goals are developed and the shared living provider provides support to the indi-vidual toward achieving identified goals. Home & Community Connections provides support to the relationship to ensure a satis-fying home life for all.
